Active Measures™ is a community diet and exercise program operating at our Bentley office to service the ongoing chronic disease needs of the community.
What we are offering
- One-on-one sessions with an Accredited Exercise Physiologist
- One-on-one appointments with an Accredited Practising Dietitian
- Group education sessions run by an Accredited Practising Dietitian
- Must be over 18 years of age
GP to complete the Active Measures Patient Referral Form and consider the following:
a) If your patient has a chronic or terminal medical condition with a GPMP (item 721) and TCA (item 723), please provide a Referral Form for Individual Allied Health Services.
b) Those with private health ancillary cover may be entitled to a rebate through their health fund. Private fees will exist for those who are not eligible for Medicare or private health rebates. For private patients please provide an Active Measures referral form which you can access here.
c) Refer to the below fee schedule for further information.

Group Nutrition Education Sessions
Group nutrition education sessions are run for all members of the community. A referral from your GP is not necessary. Topics covered in the group nutrition education sessions include:

- SUPERMARKET TOUR (including label reading)
- HEALTHY HEARTS
- BEAT TYPE 2 DIABETES
- HEALTHY EATING ON A BUDGET
- HEALTHY COOKING NIGHT
